Step 3 - Finalised Care Plan
We finalise your care plan with you, your family or advocate, making sure you are happy with every aspect.
Step 4- Identify Suitable Carer & Start Services
We identify suitable carers to support you with your care needs and arrange for you to meet them to ensure that all your needs are met. Once suitable carers have been identified, our services will commence.
