Life before Legend Integrated Care

Mrs G was diagnosed with Dementia just before the pandemic and had not been out of her house since the pandemic.Her family wanted to continue looking after her in the comfort of her own home but were struggling to support her with her wellbeing and understanding dementia.

Life before Legend Integrated Care

Living independently can be a challenge for anyone, but it can be especially for those with disabilities. Sophie, a 19-year-old with cerebral palsy, recently moved into her own flat and is navigating the ups and downs of living on her own.

 

While she is excited about the freedom that comes with independence, she also faced the challenge of achieving her dream of traveling the world without the support of family. 

Life with Legend Integrated Care 

Legend Integrated Care has successfully implemented its the social prescribing approach that focuses on connecting individuals with their local community. By understanding what her life was like before dementia and pre-pandemic.We were able to help her attend GP appointments, coffee mornings, and introduced her to a day centre where she now attends once a week. The carers also take her into her garden, which is a walk down memory lane for her.We provided the family with a carer's break cover, enabling them to attend social activities without worry.

Life with Legend Integrated Care 

Sophie's journey towards achieving her dreams has been a fulfilling one, thanks to the support she received from our lovely team of carers. We helped her connect with services that enabled her to build her skills and pursue a career in performing arts. Our support also helped her manage her diary while attending university.

 

Additionally, we signposted her to organisations that helped her understand financial management, and with our planning, she was able to tick off her bucket list item of visiting Disneyland Paris. With the support of her carers, she hopped onto the Eurostar and spent a day in Paris, meeting some of her favourite Disney characters.
